Are you a professional who has contact with carers?
If you have identified a carer, we ask you to signpost or refer them to Sheffield Carers Centre. Below is information on how to do that. 

Referral to the Universal Carers Service
When you refer a carer to the Universal Carers’ Service, they get access to:

  • Carers Advice Line: for 1:1 personalised expert information, advice and support on anything related to the caring role. One of the Carer Advisers is an Urdu/Punjabi speaker, and we use an interpretation service for other languages.
  • Carer Card: that gives discounted activities, services and products and space to write two emergency contact numbers. *
  • Group activities and workshops: that meet carers’ support needs and provide opportunities for carers to meet each other.
  • Community Connect: 1:1 telephone support for carers who are isolated.
  • Carers Café: for social contact with other carers.
  • Carers support groups: up to date information about all the groups in Sheffield.
  • Carers Enews: for regular up to date information.* Carers who do not have email receive an annual update letter. Professionals can also sign up for the Carers Enews here [hyperlink]
  • Information and resources: on the website of Sheffield Carers Centre.
  • Legal advice: session on certain carer-related topics.
  • Welcome Pack: with information about our services.*
  • Digital Resource for Carers: providing information, eLearning, resources, and the Jointly app.

*These items are sent to carers after they have been referred.

Referral criteria
• Any adult carer who lives in Sheffield (regardless of where the cared for person lives)
• Any adult carer who provides care or support for someone who is a resident of Sheffield (adult or child)
• And where the carer has given permission to be referred.

Please note, Sheffield Carers Centre does not provide or organise respite care. If this is needed for carer wellbeing, it is delivered as part of the cared-for person’s care package and should be discussed with Adult Social Care

Signposting
If a carer does not wish to be referred, we ask you to ensure the carer knows about Sheffield Carers Centre. A signposting leaflet is available here [hyperlink] and we can provide printed copies upon request.

Carers Assessments
The universal service above meets many carers’ needs, however a carers assessment may be needed in some situations, for example when the caring situation is complex or at risk of breaking down. A carer’s assessment is a structured, exploratory conversation aimed at problem-solving around unmet support needs.
