NHS Sheffield CCG have developed this survey about short breaks for carers. This is what they say about it:
“We wanted to let you know about some work we are doing to review short break services and improve the way they are allocated. Short breaks, is a break to support the carer to sustain their caring role.
We want to make sure that the services offered are meeting people’s different needs and that we are using the resources we have in the best way possible for people in Sheffield. We also want to make sure that there is a fair and consistent way of allocating breaks for everyone and that there is a clear process for doing this.
As someone who is or has been a carer for someone who needed support from health and social care, we would be very grateful if you would complete this short survey, which should take no longer than 10 minutes.”
The survey is open via Citizen Space 23 Feb – 23 March.
As an alternative to the on-line survey, carers are also invited to give their views at one of four focus groups:
Monday 19th March 10.00am – 12.00noon Reception room, Town Hall, Sheffield S1 2HH
